These Terms & Conditions (“Terms”) govern your use of the Ease DME, LLC (“Ease DME,” “we,” “our,” or “us”) website and your relationship with us as a patient or prospective patient. By using our website or services, you agree to these Terms. Please also review our Privacy Policy, which is incorporated into these Terms by reference.

01. About Our Services

Ease DME is a Medicare-enrolled, ACHC-accredited durable medical equipment (DME) supplier. We provide the following products and services:

  • Urological Supplies — Intermittent catheters, including straight tip, coude tip, hydrophilic, and sterile kits, covered by Medicare for qualifying diagnoses.
  • Ostomy Supplies — Ostomy pouching systems and related supplies delivered directly to your home on your schedule.
  • Blood Glucose Monitors & Supplies — Mail-order diabetic testing supplies, covered under applicable insurance plans.

02. Not Medical Advice

Information on our website is provided for general informational purposes only and does not constitute medical advice. Ease DME is a DMEPOS supplier, not a healthcare provider, and does not diagnose, treat, or manage medical conditions. Always consult your physician or qualified healthcare provider regarding your medical care. If you believe you have a medical emergency, call 911 immediately.

03. Eligibility & Ordering

Our products are supplied based on a valid, signed order from your treating physician (Standard Written Order) and verification of insurance coverage where applicable. All new equipment setups require prior insurance verification before equipment ships. We do not guarantee coverage or payment of insurance claims. You are responsible for providing accurate insurance information and for payment of all charges not covered by insurance.

04. Delivery, Returns & Warranty

  • All orders ship directly to your home via USPS or UPS. Tracking updates are available upon request. If a shipment arrives damaged, contact us immediately at (305) 900-4227.
  • Returns or exchanges are accepted within 14 days with receipt, in original unused packaging. Personal items, items worn next to the skin, or opened sterile goods are non-returnable unless defective.
  • New equipment carries the manufacturer’s warranty, honored under applicable Florida law. Medicare-covered items under warranty are repaired or replaced by Ease DME free of charge.

05. Billing, Insurance & Payment

  • Medicare — We may accept Part B assignment. Medicare pays 80%; you are responsible for the 20% coinsurance and any unmet deductible.
  • Medicaid — Provided upon verification and medical justification.
  • Private Insurance & Managed Care — Billed upon verification and authorization. Billing a third party does not guarantee payment; financial responsibility for uncovered charges remains with you.

06. Medicare DMEPOS Supplier Standards

As a Medicare-enrolled DMEPOS supplier, Ease DME is subject to the supplier standards at 42 C.F.R. § 424.57(c), including standards related to beneficiary notification, warranty coverage, complaint resolution, and proof of delivery. A written copy of these standards is available upon request. Ease DME is ACHC-accredited through November 3, 2028.
